The CHALLENGE trial, led by the National Cancer Institute of Canada Clinical Trials Group, is a
randomized controlled study investigating whether a structured physical activity (PA) program improves outcomes
in survivors of high-risk stage II or III colon cancer. Following adjuvant therapy, 962 participants will be assigned
to either a supervised PA intervention with behavioral support or general health education. The trial’s primary
endpoint is disease-free survival, with secondary outcomes including patient-reported measures, physical
functioning, biological markers, and cost-effectiveness. This trial aims to provide causal evidence on the
benefits of PA in colon cancer survivorship and inform future healthcare policy.
